Age-Friendly: Equipping Leaders to Ensure a Hospital-Wide Approach

Non-members: $35.
DESCRIPTION: This presentation will equip hospital leaders with a strategic framework for implementing and scaling an Age-Friendly Health System. Using the evidence-based “4-M’s” model – What Matters, Medication, Mentation, and Mobility- attendees will learn how to drive a hospital-wide transformation that addresses the unique needs of older adults.

PROGRAM OBJECTIVES:
- Describe the Age-Friendly Health Systems 4Ms framework and how it applies to the five domains of the measure.
- Identify key stakeholders’ imperative to advancing Age-Friendly care.
- Discuss integration of Age-Friendly care strategies in the acute care setting..
